Admissions Coordinator

Position Summary:

The Admissions Coordinator provides outstanding service to our prospective residents and their families at JGS Lifecare. Reporting to the Director of Admissions, the Admissions Coordinator represents the department as a professional and friendly liaison between JGS Lifecare and our prospective admissions into our facilities. The Admissions Coordinator responds to inquiries from hospital discharge planners, families and other referral sources in a professional, timely and compassionate manner. The Admissions Coordinator also works closely with the Executive Directors, Directors of Nursing and Unit Managers to frequently communicate anticipated changes regarding admissions and discharges, thus ensuring efficient coordination.

Primary Areas of Responsibilities:

Marketing Coordination

  • Build and maintain relationships with Discharge Planners, Case Managers, Physicians and all other potential referral sources.
  • Conduct tours as requested, and follow up as needed.
  • Follow up with referral sources.
  • Daily follow up on all active and pending inquiries.
  • On site screening, as needed.
  • Weekend on-call for admissions.

Pre-Admission Assessment and Service Coordination:

  • Respond to all referrals promptly and professionally.
  • In conjunction with facility requirements/abilities, clinically access patients to determine if the patient meets admission criteria.
  • Make contact with referral sources as a representative of CJL. Review charts, and assist with any questions from prospective residents, families, and other referral sources.
  • Obtain the necessary information (medical and financial) to qualify patients for admission.
  • Facilitate patient/resident placement based on clinical services needed, bed availability, patient/family preference, etc.
  • Troubleshoot areas of Admission, Public Relations, pre-certification and physician follow-up in conjunction with Admissions Director.
  • Provide support and resources as appropriate or requested, regarding alternative services, placement options, etc.
  • Data collection for all referrals.

Admission and In-House Coordination:

  • Responds to inquiry calls from hospital discharge planners, families, and other referral sources. Conducts facility tours. Manages the admission process by maintaining bed availability and facility services information at all times. Manages the inquiry process professionally, timely and with appropriate follow-up.
  • Alerts appropriate department heads and building staff of projected changes; i.e. admissions, bed changes, and discharges daily through established communication systems.
  • Recommends admission decisions to Admission Director and/or facility management.
  • When admissions determinations are made, communicates with nursing and housekeeping about appropriate room and bed selection
  • Ensures appropriate admission paperwork and signatures are obtained from residents or responsible parties prior to admission.
  • Communicates special needs of new admits to staff to ensure a smooth transition.
  • Ensures daily that referral sources and admissions data are entered into the automated referral system.
  • Maintains a working knowledge of Federal and State regulations and reimbursement guidelines (Medicare and Medicaid). Assists with managed care referral process
  • Keeps all patient/resident information confidential.
  • Signs in all patients at point of admissions.
  • Performs other duties as assigned.

Primary Qualifications:

  • Associates and/or Bachelor's Degree in healthcare or related field, or licensed clinician in good standing, currently licensed by state in which the services are provided.
  • Knowledge of relevant software applications including MS Office, preferred
  • Must be proficient in use of email and internet
  • Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel, and the general public
  • Must be able to read, write, speak and understand the English language
  • Must possess a high degree of interpersonal relations skills with the capability of relating to a variety of people and personalities. Must use good judgment and take initiative when necessary.

Hours:

  • Full-time, 40 hours
  • Monday Friday 9:00AM 5:00PM

Primary Responsibilities:

  • Makes the initial assessment and completes the appropriate forms necessary for the start of care.
  • Develops a written plan of care.
  • Identifies patient and family needs for other home health services and makes the referral for same when appropriate.
  • Initiates appropriate preventative and rehabilitative nursing procedures for the patient.
  • Coordinates and integrates services being provided with the patient, family, and other involved in the care plan.
  • Provides instruction and counsels the patient and family in meeting nursing and related needs.
  • Visits and completes an assessment and reassessment of assigned patients receiving homemaker-home health aide based on the patient care plan and supervises the care provided by him/her.
