The nonprofit San Francisco Campus for Jewish Living serves a significant number of patients and long-term care/skilled nursing residents each year in its Jewish Home & Rehab Center. Non-residential care is offered through our short-term and rehabilitation services unit for patients who require specialized interdisciplinary rehabilitation to recover from surgery or an acute illness, and through our acute geriatric psychiatry hospital. With a complement of close to 650 compassionate, experienced clinical and allied health personnel, SFCJL provides older adults with a variety of life-enriching healthcare programs and services in a welcoming, stimulating environment.


SFCJL�s transformed campus includes the new Lynne & Roy M. Frank Residences (assisted living apartments and memory care & support assisted living suites), and Byer Square, a non-residential community of programs, services, and support.

Position Overview:

The Intake Coordinator � Admissions position is primarily responsible for preparing and completing admissions paperwork for admissions. This position is an on-site position.


Essential Functions:



  • Create admissions paperwork for all incoming admissions. This includes relevant payer information (patient responsibility form), as well as general information about the Home.

  • Create name tag and information sheets for incoming admissions. Information sheets include

  • Care Team contact information and Care Conference schedule.

  • Perform room check to ensure room is prepared for patient�s admission. This includes checking the working order of the phone, call light, television, and other aspects of the room.

  • Complete admissions paperwork, uploading appropriate documents to EMR.

  • Welcome newly admitted patients and assist them to their room, familiarize patients to call light, TV channels, phone, fan, and lighting.

  • Provide patient/families with general customer service (when needed).

  • Take patient photograph and upload to EMR.

  • Provide tours of the units for potential patients/families.

  • Assist team to answer call lights in a timely fashion and assists patients in getting needs met.

  • Monitor cleanliness of both F-building elevators and entry way floor. Reports cleaning requests to housekeeping & repair requests to maintenance.

  • Perform other duties as assigned.


Qualifications:

High school diploma or equivalent, required; Associates degree, preferred

Four years administrative assistant experience, preferred

Two years fast-paced office environment, preferred

Three years restaurant or retail environment, accepted in lieu of office experience

Demonstrated proficiency with MS Office, including Outlook, Word, Excel

Demonstrated high level of interpersonal skills to handle sensitive and confidential situations

Demonstrated ability to build rapport quickly with peers and customers

Excellent customer service skills

Excellent communications skills, both written and verbal

Ability to read, write, communicate, and comprehend verbal and written instructions in English

Ability to be accurate, concise, detail oriented and make independent decisions
