Pay Range: $75,000-$80,000/year

Position Overview:


JFS is looking for a compassionate, mission-driven individual to lead the case management team as the Director of Case Management. In this role, you have the opportunity to use your evaluation skills and collaboration experience to make a meaningful difference in your community and help change lives across San Diego. The Director of Case Management leads a robust case management team ranging from community case management, Project Sarah- intimate partner violence support services, enhanced case management and resource navigation. This position reports directly to the Senior Director of Nutrition and Aging and works closely with the Director of Nutrition, Director of Aging Connections, Director of Transportation and the rest of Jewish Family Service.


Responsibilities:


Care Coordination



  • Oversee coordination of care plans for enrolled individuals

  • Monitor caseloads and progress towards goals

  • Foster trauma informed and culturally competent case management

  • Lead and supervise the case management team, including recruitment, training, performance evaluation, and professional development

  • Monitor and improve workflow processes to optimize case management effectiveness and enhance the quality of care 


Contract Compliance



  • Ensure program is fulfilling contract deliverables and meeting compliance requirements

  • Develop and implement quality assurance measures, tracking case management outcomes to ensure effective care delivery

  • Regularly review and analyze case data to identify areas for improvement and implement changes as needed


Resource and Financial Management



  • Collaborate with the finance department to review and report on budget

  • Track program expenditures and revenue

  • Work with accounting and third-party billers to ensure accurate and timely reimbursement for health care contracts


Internal and External Collaboration



  • Foster internal connections with JFS services to ensure

  • Foster partnerships with other organizations to fulfill unmet needs


Skills/Experience/Abilities That Are a Must-Have:



  • Master’s level education such as MSW

  • At least three years of supervisory experience

  • Strong leadership and team management skills

  • Excellent communication and interpersonal abilities to work with stakeholders (internally and externally) and those we serve

  • Analytical skills with the ability to interpret case data and implement process improvements

  • Proficiency in case management software and electronic health records (EHR)
