JOB TITLE: Community Based Master’s Prepared Clinician (Mobile Therapist)
SUMMARY:
The Community based Master’s Level Clinician will work under the direction and clinical supervision of the Deputy Director of Programming. The Community Based Clinician is responsible for providing assessment and therapy services for children, adolescents and adults in the New Castle county community. The Therapist is responsible for coordinating, managing, supporting and facilitating appropriate services to meet the needs of individual clients. This position requires coverage of New Castle counties. It is 100% community based-- you will be in the homes of clients and/or in community centers. You will be reimbursed for mileagebased on IRS standards. .
DUTIES and RESPONSIBILITIES:
- Develop and document initial assessments of individual treatment needs by way of one-on-one sessions and use of any records that are sent by referral agencies in the client home.
- In collaboration with other resources--including family members, agency representatives, other agencies involved in the individual's treatment--using behavioral health assessment tools and following guidelines established by regulatory and practice requirements, develop treatment plan goals for assigned individuals
- Maintain progress notes and progress reports, and modify treatment plans according to changes in client status
- Provide clinically appropriate treatment through individual counseling consistent with the treatment plan goals
- Utilize individuals' natural and professional supports as relevant to assist individuals in meeting their treatment plan goals
- Assure continuous reimbursable sessions by maintaining the required number of sessions as outlined in the offer agreement.
- Ensure that all required documentation is completed to standards prior to inspections by payer or regulatory agencies
- Interface with other Jewish Family Services clinical personnel, including therapists, psychologists, psychiatrists, and Certified Peer Specialists relative to the individual's overall treatment plan, in support of the overall outpatient program
- Attend internal team meetings and other outside meetings as directed by the Clinical Director or designee with the goal to support the clients’ needs in reaching their treatment plan goals
- Perform other duties as assigned
REQUIREMENTS
- Must have a Master’s in Human Services or Social Services
- Self-motivated, warm and genuine, and can provide excellent clinical care in an outpatient private practice setting.
- Demonstrated ability to work effectively as part of a team
- Ability to drive short and long distance in a day and ability to get in and out of the vehicle comfortably and safely.
- Must be able to enter homes that may have stairs and no elevator.
- Must possess excellent customer interaction, collaboration, presentation, and written and verbal communication skills
- Demonstrated experience with Microsoft Office applications, including Word and Outlook
- Must be able to utilize Electronic Health Record (EHR) / Electronic Medical Record (EMR)

SALARY RANGE: $50,000-$55,000 based on experience
