Reports To: Enhanced Care Management and Community Supports Program Manager

Pay Range: $22-$24/hour

Position Overview: 

JFS is looking for a compassionate, mission-driven individual to join the Enhanced Care Management and Community Supports team as the Outreach Specialist. In this role, you have the opportunity to use your communication skills and client service experience to make a meaningful difference in your community and help change lives across San Diego. The Enhanced Care Management and Community Supports Programs helps coordinate care for Medi-Cal members with chronic health conditions, who have high health care needs, or are experiencing homelessness. The Outreach Specialist plays an integral role on the team and supports the entire agency by conducting initial telephonic and in-person outreach to potential clients, verifying participant eligibility, and following up contact with program participants. The Outreach Specialist will be making a high number of phone calls on a regular basis, will attempt to contact clients at their homes, and other community settings, and organize and facilitate mail outreach campaigns. This position reports directly to the Enhanced Care Management and Community Supports Program Manager and works closely with the Enhanced Care Management Resource Navigators and Community Supports Housing Navigators.

Responsibilities:

  • Conduct initial outreach to potential Enhanced Care Management clients via telephone call, home visits and visits to other client identified community locations
  • Determine program eligibility and schedule follow up assessment appointments
  • Organize and facilitate mail outreach campaigns
  • Conduct electronic program eligibility verifications
  • Accurately track and input program data in a variety of software platforms
  • Assure all client information remains confidential and secure
  • Assist with program participant follow up phone calls and documentation as needed
  • Support program with special projects as needed
  • Other duties as assigned by Enhanced Care Management and Community Supports Program Manager

Skills/Experience/Abilities That Are a Must-Have:

  • High School Diploma or bachelor’s degree (in a human services field) preferred
  • Bilingual and bicultural candidates are highly preferred
  • Must have a valid driver’s license and a reliable transportation for regular travel around San Diego County
  • Ability to work independently and manage daily tasks efficiently
  • Comfort working in an environment that has multiple languages spoken and a diverse service population.
  • Excellent computer skills, including knowledge of Microsoft Office, Adobe Suite and client management software.
  • Ability to apply good judgment and critical thinking to carry out instructions and make decisions within the scope of authority
  • Excellent organizational skills
  • Ability to learn and utilize new computer applications
  • Reliable, detail-oriented and initiates hard-work without being prompted

Skills/Abilities We’d Like You to Have:

  • Fluency in Spanish preferred
  • At least one year of case management experience, outreach and community-based visit experience preferred
  • Prior experience in the non-profit sector preferred

Physical Requirements:

  • Ability to sit/stand and work at their workstation while performing their job duties
  • Ability to drive to conduct outreach in the community
