Parker Jewish Institute Hospice and Home Health Care is seeking compassionate and motivated Community Registered Dietitians to join Parker's Certified Home Health Agency (CHHA) on a per visit basis.

Parker Jewish Institute Hospice and Home Health Care offers the finest in home health and hospice services from an interdisciplinary team of health care professionals. Parker Jewish Institute for Health Care and Rehabilitation is a not-for-profit adult health care system comprised of multiple health services servicing the Metropolitan New York Area, including Long Island, Brooklyn, Queens, Bronx, Manhattan and Westchester.

The Dietitian at Parker Jewish Institute's Certified Home Health Agency is responsible for the assessment, delivery and ongoing monitoring nutritional care plan services to home care patients. They are also responsible for the coordination of services through the Nurse Coordinator and for timely documentation.

This position requires the full understanding and the active participation in fulfilling the mission of Parker's Certified Home Health Agency to deliver exceptional care and to enhance the quality of life of our patients.

QUALIFICATIONS/EXPERIENCE:

• Degree in Food and Nutrition
• Registered Dietitian
• Experience with clinical nutrition care in hospital or nursing home, or similar clinical environment
• Prior experience in a community setting strongly preferred
• Tech savvy, with proficiency in AllScripts or another electronic medical record (EMR)
• Access to reliable transportation a must
• Fully Vaccinated against COVID-19
