We are an innovative social service agency and community-based organization that has served individuals and families throughout Alameda and Contra Costa counties since 1877. We reunite and strengthen families, care for the mental and physical health needs of people impacted by trauma, support healthy relationships between young children and their caregivers, provide legal representation to immigrants, and resettle refugees who seek to start their lives anew. JOB SUMMARY 

The Aging Services Case Manager provides care consultation, case management, and support services to older adults, adults with disabilities, Holocaust survivors, and their caregivers in Alameda and Contra Costa counties. This role manages key programs such as the Information and Referral Line, home care coordination, community support groups, and the Rossmoor Respite Program.

We aim to help older adults and adults with disabilities live independently and with dignity in their homes or communities for as long as possible. Our person-centered approach ensures that care plans respect each client's preferences and autonomy, empowering them to maintain control over their lives.

Recognizing the vital role family caregivers play, we offer tools, resources, and emotional support to lighten their load and guide them through the challenges of caregiving. This improves the care experience for both caregivers and those they support.

This position is designed to grow into a Fee-for-Service Aging care management role, focusing on providing direct services on a fee-for-service model in the future.


DUTIES AND ESSENTIAL JOB FUNCTIONS 

Information and Referral Line:

  • Manage incoming calls on the Information and Referral Line, providing resource guidance, referrals, and tracking services to ensure follow-up and service continuity.

Care Coordination and Consultations

  • Provide short-term case consultations and ongoing care coordination services to older adults and adults with disabilities. This involves conducting assessments and developing individualized care plans that reflect client preferences, ensuring measurable and cost-effective outcomes. Additionally, actively plan and implement Fee-for-Service Aging Care Management Services, offering direct care services such as assessments and consultations on a fee-for-service basis to promote flexible, client-focused care.

Program Oversight:

  • Oversee the home care program, ensuring responsiveness to client needs and maintaining quality control.
  • Coordinate the operations and client engagement of the Memory Café and Rossmoor Respite Program.

Holocaust Survivor Support:

  • Deliver trauma-informed care and case coordination services for Holocaust survivors, addressing their unique needs.


MINIMUM QUALIFICATIONS 

  • Master’s degree in Social Work, Gerontology or a related field.
  • 3-5 years of experience working with older adults, adults with disabilities, or in related care coordination roles.
  • Experience working in a non-profit, community service, or health care setting is a plus.

REQUIRED COMPETENCIES 

  • Knowledge of aging-related issues, including geriatric care and dementia care.
  • Strong interpersonal and communication skills, with the ability to work effectively with clients, families, and community partners.
  • Demonstrated cultural competence, with experience working with individuals from diverse nationalities, ethnicities, and religious backgrounds including knowledge of Jewish culture and community.
  • Understanding of trauma-informed care practices, particularly for Holocaust survivors and those affected by trauma.
  • Excellent organizational, time management, and documentation skills.
  • Proficiency in Microsoft Word, Excel, and experience with client tracking systems.
  • Familiarity with community resources and services in Alameda and Contra Costa Counties

COMPENSATION & BENEFITS
The compensation range is $75,000-$85,000 per year, depending on licensure status.
