How you can make a bigger difference: 


 


PURPOSE: 


 


The Supervised Independent Living Program (SILP) provides transitional living environment for older youth who are ready to move towards independent living but could benefit from a supportive environment to build the skills required to live independently. The SILP provides resources for youth 18-21 years old who are experiencing challenges as they transition to adulthood.


 


POSITION OVERVIEW:


The Case Manager provides support, counseling, case planning and works with the youth towards independent living.


 


 


KEY ESSENTIAL FUNCTIONS (List the functions that are essential to achieve the goals):


 


 Client Engagement:



  • Carries a caseload of clients and provides services to those residents within the scope of program regulations and agency mission.

  • Evaluates and assesses independent living skills and supports clients in meeting individual goals.

  • Teach the clients how to advocate for themselves with service providers by modeling advocacy skills.

  • Links clients to community-based treatment and collaborate with other services providers when coordinating services within the Jewish Board as well as with community providers.

  • Practice person-centered and collaborative service planning with clients, also ensuring service plans are completed and signed by client according to regulatory guidelines and standards.

  • Interprets and explains agency policies and procedures to client.

  • Run groups under supervision of the clinical supervisor.

  • Supports residents toward the goal of medication management independence, including appropriate person-centered interventions.

  • Accompanies residents to appointments as needed.

  • Makes weekly home visits during business hours as well as after hours as needed.

  • Attends court for the clients.

  • Other tasks as assigned.


 


Team Process:



  • Participates in intake, assessment, discharge, and follow-up conferences with service providers, ACS/ OCFS where needed.

  • Participates in regular staff meetings and training.

  • Travel to offsite meetings and trainings.


 


DOCUMENTATION:


 



  • Files incident reports as appropriate.

  • Complete paperwork, including case notes, FASPs, PHR’s and court reports, in a timely manner.

  • Complete monthly home inspection.

  • Participate in audit preparation.

  • Ensures progress notes meet monthly billing documentation requirements, if applicable.

     EDUCATIONAL / TRAINING REQUIRED (List all that is required to achieve this position):


 



  • A bachelor’s degree is required, master’s degree in human services or related field preferred


 


ADDITIONAL QUALIFIICATIONS NEEDED:



  • Willingness to work with young adults living with the impact of trauma and experience mental health challenges.

  • Compassion and respect for vulnerable individuals.

  • Patience and understanding for the sometimes-slow process of stabilization and recovery.


 


COMPUTER SKILLS REQUIRED (List the computer skills needed):


 



  • Familiarity with working in windows, Microsoft word, excel, Teams etc.

  • Ability and willingness to learn new systems and application software.

  • Familiarity with Connections (ACS record), preferred.

  • Familiarity with telehealth.


 


VISUAL AND MANUAL DEXIERITY:


 



  • The candidate should be able to read documents for analytical purposes such as computer information, Excel spreadsheets, software, computer graphic design programs, etc.

  • Limited applications of manual dexterity and hand-eye coordination.


 


WORK ENVIRONMENT / PHYSICAL EFFORT:



  • Travel in the community with residents.

  • Willingness to travel between locations.

  • Walk up and down stairs
