Children's Case Manager: JFCS is seeking an enthusiastic and empathetic Case Manager to assist children who may be receiving behavioral-health services on the path to a more self-sufficient lifestyle and/or family reunification. The Case Manager will also provide guidance and support to the families/caregivers of these youth as needed.









Responsibilities






  • Provides Case Management services to assigned clients.
  • Follows required guidelines for monthly contact, quarterly home/community visits, attending medication checks, etc.
  • Assists clients in finding appropriate services and works with children to decrease behaviors and improve stability in the community & home.
  • Responsible for coordinating and facilitating Child & Family Team (CFT) meetings.
  • Collaborates with clinical team to ensure that Integrated Service Plans, CFT meetings, and all required documentation are completed accurately and in a timely manner.
  • Collaborates and coordinates services with other involved agencies, such as the Department of Child Safety (DCS), Probation/Parole Departments, Division of Developmental Disabilities (DDD), schools, and other providers.
  • Completes referrals, prior authorizations, and other paperwork as required to secure appropriate services for clients.
  • Will inquire about the client’s health & wellness needs to assist in coordinating primary care or other supportive services with appropriate JFCS staff or external parties to ensure the client’s whole health is addressed.








Qualifications






-Meets one of the following education/experience requirements:

  • High School Diploma/GED required and 4 years of Behavioral Health experience.
  • A.A. Degree and two years of Behavioral Health experience
  • Bachelor’s Degree in Social Work, Psychology, or related field.

-Additional qualifications:

  • Level 1 Fingerprint Clearance Card without Restriction(s)
  • Meet Agency requirements for MVR (Motor Vehicle Record) Report
  • Reliable transportation
