The CHHA Department in Community Services Division of The New Jewish Home is seeking Per Diem Speech Language Pathologists to join our team!
The qualified candidate will join the CHHA Department in providing home care service to our clients in their homes. Specifically, the Speech Language Pathologist will conduct home visits and will provide speech therapy assessments and treatments to our clients.
Responsibilities:
- Provides Speech-Language and Swallowing screening and evaluation of residents referred by the physician.
- Provides therapeutic intervention for patients/residents with voice, speech, language, and swallowing disorders when indicated.
- Works with patients/residents and/or families concerning appropriate Speech-Language Pathology therapy discharge plans.
- Prepares written and oral reports including: evaluation, progress notes and discharge summaries.
- Participates in meetings and actively collaborates with patients/residents and member of the interdisciplinary team to achieve common goals.
- Maintain appropriate departmental records.
- Participates in the educational activities of the home.
- Assists in the upkeep of the work areas, equipment and supplies.
- Maintains standards of practice by keeping current with new developments in the field and applying them as appropriate.
- Performs other duties as required.
