JF&CS is now seeking a full-time Geriatric Care Manager.

  

PURPOSE

Jewish Family & Career Services of Atlanta (JF&CS) provides health, career, and human services to support and enhance the well-being of individuals and families across all ages, faiths, cultures, and lifestyles. Its more than 30 programs serve a variety of populations, including children, teens, parents, and older adults. JF&CS is now seeking a Geriatric Care Manager for Aviv Older Adult Services.


Aviv Older Adult Services, a program of JF&CS, provides a comprehensive, holistic approach to offering solutions and resources for individuals and their families struggling with the challenges of aging.  We are seeking a full-time Geriatric Care Manager to:

To provide clients who have significant life challenges and/or mental illness with the supports necessary to help them live more securely and independently in the community. 


To provide geriatric care management services for family caregivers and care recipients as a member of the Supportive Services Team. 


ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.

  • Provide private pay geriatric care management services to older adults and their family caregivers, as assigned, including:
  • Assess in home or in office to determine what is needed to maximize older adult’s quality of life. 
  • Complete evaluations and referral recommendations for where to turn and what to do.
  • Design, implement and manage a comprehensive plan of care to relieve clients’ uncertainty, teach clients about available services, and help them understand the language and programs involved in eldercare.
  • Educate family caregivers and older adults concerned about appropriate levels of care and available resources.
  • Conduct family meetings to provide a safe space and a neutral party to listen to family concerns and help work toward resolving disagreements.
  • Follow up and triage new potential clients to the appropriate GCM based on capacity and skill set to match client needs.
  • Coordinate services, appointments and follow-up based on individual needs and decisions.
  • Provide assistance with living-arrangement transition.
  • Assist with and advocate for older adult clients at doctor’s visits and hospital stays and provide a review of clients’ care.
  • Assist with screening, arranging, and monitoring services.
  • Maintain all client files in accordance with CARF regulations and funding requirements, including consents, assessments, client goals, progress notes and discharge/care transition summaries, as applies.
  • Facilitate Caregiver Support Groups and trainings, as needed, for partner senior living communities.
  • Enter and maintain all required information and documentation in Electronic Health Records software (InSync) in a timely manner.
  • Conduct in office and/or in home visits with private pay family caregivers and care recipients.
  • Participate in supervision with the manager of Caregiver Support Services and clinical supervision meetings as scheduled.
  • Carry and take emergency client calls via   on-call phone for stipend on a two-week rotating basis.
  • Attend Aviv staff meetings and caregiver workgroup meetings, as needed, and work with Aviv staff as part of a team.
Requirements
  • The candidate must demonstrate client-centered values and a commitment to working to enhance the lives of older adults and their families.
  • Excellent knowledge base and comfort level working with older adults and their family caregivers. 
  • Solid working knowledge of community resources for older adults and familiarity with provisions and eligibility for entitlement programs.
  • Knowledge of cognitive behavioral and dialectical behavioral therapy techniques considered a plus.
  • Excellent skills as a client advocate to access resources.
  • Able to organize, mediate, be creative, multitask, convey empathy and be willing to learn. A good team player. Able to plan and help clients reach goals. Good use of the English language and able to communicate clearly. Must be able to manage time effectively, maintain boundaries and cope with crises. Must have valid Georgia Driver’s License and insured personal vehicle. Other duties as assigned.

  • The employee must occasionally lift and/or move up to 10 pounds. Specific vision abilities required by this job include close vision and the ability to adjust focus.

EDUCATION and/or EXPERIENCE:

  • MSW or other human services degree, with LMSW/LCSW or RN a plus.
  • Experience providing geriatric care management services or similar support to elders and family caregivers.
  • Knowledge and experience working with resources relevant to working with elders and family caregivers in Atlanta, GA. 

Candidates who are selected for employment must successfully complete a pre-employment background screening as well as a pre-employment drug screening. Employment will be contingent upon the satisfactory completion of both the background check and the drug screening. Additionally, if applicable, tuberculosis testing (TB) and fingerprinting may also be required.
