We are looking to immediately hire a Per Diem Social Worker to join our our amazing Home Health team!
Territory: North and East King County
The Social Worker will be responsible for providing a range of social work services including: comprehensive psychosocial assessments, developing a plan of care in collaboration with the team, crisis and therapeutic interventions, resource referral and coordination with community providers. The Social Worker may provide care in private residences, Adult Family Homes, Independent and Assisted Living, Skilled Nursing Facilities.

-Master's Degree in Social Work from an accredited school
-2 years Social Work experience (1 year Hospice and/or Home Health experience preferred)
-Strong verbal and written communication skills
-Computer skills for charting and documentation
-Valid driver's license , auto insurance, dependable vehicle
