About Us
UofL Health – Frazier Rehabilitation Institute began its 60-year history in 1950 when Amelia Brown Frazier, ailing and in need of rehabilitation from a car accident in 1929, was traveling once again to New York to receive care at the Rusk Institute, a leader in the then-emerging field of physical medicine and rehab. A woman of vision saddened that the best care was not available in Kentucky, she set out to create a comparable facility in her hometown. Using her substantial personal wealth and influence, Frazier was the catalyst for the creation of The Rehabilitation Center, Inc. in 1951. In 1984, the name was changed to the Amelia Brown Frazier Rehabilitation Center to honor her leadership. In 1994, following a long relationship with UofL Health – Jewish Hospital, Frazier Rehab Institute merged with then Jewish Hospital HealthCare Services. In 2019, Frazier Rehab Institute became part of UofL Health.

Job Summary:
As a Rehabilitation RN, you will collaborate daily with physicians, therapists, and case managers to achieve the best outcome for patients who have experienced life altering events. In this role you will ensure appropriate plans of care are developed and implemented to meet patient needs for rehabilitation.
Area of Focus
• 10 bed unit with low patient ratio
• Working with adults 18 years and older who are receiving rehab from having a transplant, recovering from multi trauma accident, Dialysis, and VADs.
• Assumes responsibility for acquiring knowledge and skills necessary for patient care in the identified practice setting.
• Performs assessment and analyzes patient problems and conditions using critical decision making to determine need for standard, urgent or emergent interventions.
• Implements and/or modifies plan of care using assessment data, standards of care and evidence based practice inclusive of nursing care coordination, appropriate patient and family education, and discharge planning. Facilitates interdisciplinary partnership in patient care.
• Integrates ethical provisions in nursing practice and delivers care in a manner that preserves and protects patient autonomy, dignity and rights.
• Evaluates progress towards attainment of desired patient outcomes.
• Demonstrates effective on-going observations and reassessment of patient/family and their response to the interventions and changes in condition.
• Practices patient experience best practices of AIDET, Hourly Rounding, Use of the White Board as minimal expectations. May also practice bedside shift report and other best practices required by facility.
• Documents assessments and interventions in accordance with hospital policy and standards of care.
• Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors. Demonstrates knowledge of proper identification of and reporting of medical/healthcare errors. Protects the patient in the event of a medical/health care error.
• Utilizes resources appropriately and follows facility guidelines for documentation and charging of medical supplies and equipment.
• Assigns or delegates tasks to designated support personnel based on needs and conditions of the patient, complexity of the task, predictability of the outcome and facility policy.
• Collaborates with new employees and peers to support orientation, training and development opportunities. Supports the profession of nursing through support for nursing student clinical activities, as appropriate.
Education / Accreditation / Licensure (required & preferred): Current RN license in the State of Kentucky; BSN preferred. BLS required. See attached grid for required certifications.
New hire RNs have 2 years from hire date to enroll in BSN program and it must be completed within 5 years of hire. See attached grid for additional certifications required.
Experience (required and preferred): Graduate of an accredited school of nursing required.
Job Requirements
• Registered Nurse credentialed from the Kentucky Board of Nursing obtained prior to hire date.
• BLS required from American Heart Association.
• Associate degree from accredited nursing program
Preferred Requirements
• Bachelor’s Degree from accredited nursing program
