Under the supervision of the Program Director of the West Hollywood Comprehensive Service Center, the Mental Health Social Worker provides targeted and intensive clinical case management services to seniors and younger disabled adults with an emphasis on mental health disabilities. Social worker will interface with clinical social workers, volunteers, and staff within the agency.

 Qualifications:                               

  • Master’s degree in social work or related field from an accredited school and license eligible
  • Unrestricted CA Driver’s License and proof of auto insurance
  • Excellent clinical, written, and verbal communication skills. 
  • Knowledge of community resources and the aging process
  • Experience working with clients with hoarding disorders is highly desired
  • Experience working with seniors, caregivers, and people with mental health challenges
  • Prior experience working with the LGBTQ+ community is a plus
  • Knowledge of safety issues when working with at-risk clients
  • Proficiency in common word processing, database software applications and the ability to learn new computer applications
  • Flexibility and ability to work autonomously
  • Stellar time management skills required
  • Candidates must have interest and demonstrated ability to:

  • Must pass a criminal background check (Live Scan fingerprinting) prior to beginning employment 

Responsibilities/Essential Functions: 

  • Provide intensive case management to seniors and younger disabled adults with moderate to severe mental health problems residing in West Hollywood, including those with Hoarding Disorder
  • Conduct in-depth intakes and assessment covering psychological, social, physical, and environmental concerns
  • Provides short-term or ongoing psychotherapeutic support to clients and families as needed
  • Identify and develop family and other informal support systems for the client
  • Provide outreach to the West Hollywood community, presentations, participation in collaborative service provider meetings and community events
  • Conduct in-home assessments to provide resources and concrete services 
  • Crisis intervention
  • Collaborate with the local municipality of the City of West Hollywood for the coordination of services for clients
  • Complete client documentation in a timely fashion
  • Participation in weekly supervision and care plan meetings
  • May be required to be a Field Work Supervisor and/ or preceptor for Social Work students
  • Other related duties as required
  • Hours towards licensure available but not guaranteed
