To manage and oversee services provided for family caregivers and individuals through Case Management grant funded programs, the Geriatric Care Management Services, and other related supportive services.

EDUCATION and/or EXPERIENCE:
Master’s degree in an area of human services preferred, with related experience working with elders and family caregivers, as well as strong knowledge of community resources. Candidate must have experience supervising others and managing programs.
Candidates who are selected for employment must successfully complete a pre-employment background screening as well as a pre-employment drug screening. Employment will be contingent upon the satisfactory completion of both the background check and the drug screening. Additionally, if applicable, tuberculosis testing (TB) and fingerprinting may also be required.
